An increasing emphasis on safety and quality standards, the need for better monitoring, documentation and reporting is all at the forefront of Residential Aged Care Facility (RACF) management priorities. It is now more vital than ever to always have the most up-to-date, accurate information, easily available and accessible to all healthcare providers.

Clear, concise, and relevant patient information reduces the risk of mistakes in medication administration and ensures a smooth handover between facility staff and healthcare providers. Detailed data is key to improving processes, ensuring quality patient care and better health outcomes for your residents.

With that in mind, the ÍøÆØ³Ô¹Ï team has expanded the reporting repertoire in its latest MMS® (Medication Management Software) and MedSig® e-signing software updates. Here are a few of the essential reports we believe will change your (work) life for good.

1. Psychotropic Medication Report

Currently a hot topic with the Royal Commission into Quality Care Standards, this report summarises, graphs and details all psychotropic medications taken by residents at any point in time. With the ability to categorise by Resident or by Drug Class, this report allows you to quickly identify and monitor the use of psychotropic drugs in your facility. Your pharmacy can provide an excel version to simplify your monthly self-assessment reporting processes.

2. Psychotropic Medication Changes Report

The Psychotropic Medication Changes Report lists all changes of psychotropic medications within a specified date range. This report allows you to clearly identify changes overtime and provides evidence of minimising chemical restraints according to the Quality and Safety Commission.

3. Falls & Confusion Report

The Falls and Confusion Report looks at a resident’s current medication profile and uses a scientifically proven mechanism to identify those with a high anticholinergic burden index. This is essential to understanding the needs of new residents upon admission to the Residential Aged Care Facility.

This report can also be generated per section, for instance if an existing patient is being transferred between wings, you can update new and existing staff to make sure all are aware of the vulnerabilities of patients in their care.

4. Polypharmacy Report

The Polypharmacy Report provides a summary of the number of medications a resident is currently taking. This can be produced to include or exclude PRN medications for your convenience. The report acts as a prompt if a resident is taking over 9 medications, and a referral for a Residential Medication Management Review (RMMR) may be suggested.

This is an essential report to ensure resident medications, interactions and side effects are being regularly monitored to ensure the best health outcomes for the patient.

5. Family Conference Report – Medication Care Plan

When preparing for family conferences or assessing medication care plans, this report gives you all the detail you need to review patient care. The report consists of three parts;

• The patient’s complete and current medication profile.
• Medication information summary, including the use of medications and possible adverse reactions; and
• Clinical correlation report, which details the history of dosage changes of the patient’s medications.

This report gives you all the data and detail to confidently speak with patient family members on the quality of care their loved one is receiving at your Aged Care Facility.
