The prestigious annual list, published by The Australian Financial Review and Boss Magazine, is based on a rigorous assessment process managed by Australia’s leading innovation consultancy, Inventium, in conjunction with a panel of industry expert judges.

سԹ was honoured to rank #4 on the healthcare list, for the second year in a row, from over 600 nominated organisations across Australia and New Zealand. The assessment measures a top innovation implemented in the past two years. Specifically, the judges look at how valuable the problem is that the innovation is solving, the quality and uniqueness of the solution, and the level of impact that the innovation has had. Inventium also assesses internal elements such as innovation culture, strategy, resources and process, which demonstrate a sustainable and repeatable approach to innovation.

Theis a consolidated list of medicines prepared by a pharmacist and uploaded to a patient’s My Health Record.The PSML will compile both prescription and non-prescription medicines, including over-the-counter and complementary medicines, such as vitamins and herbal remedies.

The overview the PSML will provide for healthcare providers will enable them to make more informed prescribing and treatment decisions and assist in reducing medication mishaps.

Particularly relevant at times of transitions of care, theis a comprehensive list of medicines at a specific point in time, such as on discharge from a hospital when a patient’s care is being transferred to another health care provider – a common occurrence in Australia’s elderly population.

The winners of Australia’s peak international design awards – the highest honour for design and innovation in the country were announced today during the 2020 Good Design Week. 

±ٱ-貹® 6 Dose (Parkinson’s) received a prestigious Good Design Award Gold Accolade in the Communication Design Packaging category in recognition for outstanding design and innovation.

Managing Director Gerard Stevens AM says “We are incredibly proud to win this award. Medication management is a major concern for someone living with Parkinson’s and their families and ensuring adherence is key to improving the quality of life for patients,” said Gerard Stevens AM, Managing Director of سԹ. “This prompted سԹ, in partnership with Parkinson’s NSW, to develop a special medications folder specifically for Parkinson’s drugs.

Almost 14,000 Australians are diagnosed with Parkinson’s every year. While the disease cannot be cured its symptoms can be managed with a combination of medication and multidisciplinary support, enabling people with Parkinson’s to live independent and productive lives.

However, once medication is prescribed, it is lifelong and requires taking medications on time, every time. Current treatment largely relies on prescription drugs administered in tablet or capsule form and can involve multiple daily doses and complex dosing regimens. This complexity plays a part in poor compliance among patients. Some studies suggest non-compliance is as high as 67 percent.

A brief guide to سԹ’s updated reporting features. 

An increasing emphasis on safety and quality standards, the need for better monitoring, documentation and reporting is all at the forefront of Residential Aged Care Facility (RACF) management priorities. It is now more vital than ever to always have the most up-to-date, accurate information, easily available and accessible to all healthcare providers.

Clear, concise, and relevant patient information reduces the risk of mistakes in medication administration and ensures a smooth handover between facility staff and healthcare providers. Detailed data is key to improving processes, ensuring quality patient care and better health outcomes for your residents.

With that in mind, the سԹ team has expanded the reporting repertoire in its latest MMS® (Medication Management Software) and MedSig® e-signing software updates. Here are a few of the essential reports we believe will change your (work) life for good.

1. Psychotropic Medication Report

Currently a hot topic with the Royal Commission into Quality Care Standards, this report summarises, graphs and details all psychotropic medications taken by residents at any point in time. With the ability to categorise by Resident or by Drug Class, this report allows you to quickly identify and monitor the use of psychotropic drugs in your facility. Your pharmacy can provide an excel version to simplify your monthly self-assessment reporting processes.

2. Psychotropic Medication Changes Report

The Psychotropic Medication Changes Report lists all changes of psychotropic medications within a specified date range. This report allows you to clearly identify changes overtime and provides evidence of minimising chemical restraints according to the Quality and Safety Commission.

3. Falls & Confusion Report

The Falls and Confusion Report looks at a resident’s current medication profile and uses a scientifically proven mechanism to identify those with a high anticholinergic burden index. This is essential to understanding the needs of new residents upon admission to the Residential Aged Care Facility.

This report can also be generated per section, for instance if an existing patient is being transferred between wings, you can update new and existing staff to make sure all are aware of the vulnerabilities of patients in their care.

4. Polypharmacy Report

The Polypharmacy Report provides a summary of the number of medications a resident is currently taking. This can be produced to include or exclude PRN medications for your convenience. The report acts as a prompt if a resident is taking over 9 medications, and a referral for a Residential Medication Management Review (RMMR) may be suggested.

This is an essential report to ensure resident medications, interactions and side effects are being regularly monitored to ensure the best health outcomes for the patient.

5. Family Conference Report – Medication Care Plan

When preparing for family conferences or assessing medication care plans, this report gives you all the detail you need to review patient care. The report consists of three parts;

  • The patient’s complete and current medication profile.
  • Medication information summary, including the use of medications and possible adverse reactions; and
  • Clinical correlation report, which details the history of dosage changes of the patient’s medications.

This report gives you all the data and detail to confidently speak with patient family members on the quality of care their loved one is receiving at your Aged Care Facility.

With monthly, weekly, and daily reporting, you can be sure your facility is empowered with the data to make informed clinical decisions.

Applying the ‘Kaizen’ principle to achieve efficient workflow and productive medication packing.

One of my guiding principles has its roots in the assembly lines and corporate meeting rooms of post-war Japan. ‘Kaizen’ describes the process of continuous quality improvement. It is the belief that everything can be improved through ongoing trial and error. A ripple from the smallest positive change can create a flow that achieves the most productive and cost-efficient processes across an organisation.

I was first introduced to Kaizen by a manufacturer who described how a machine operator becomes ‘as one’ with the machine. The operator can pick the slightest change to the rhythm or sound. Allowing them to intervene early, which can be crucial to prevent a bad batch or a machine failure.

So what does a post-war Japanese industrial concept have to do with pharmacy? Eliminating issues within any process improves productivity and output. This means fewer recalls and a corresponding reduction in costs. Quality therefore costs less. It makes sense to me.

The concept of Kaizen offers similar applications in pharmacy. We get into a zone of concentration – being totally in the moment – where we can almost intuitively identify when something is out of the ordinary, irrespective of however slight an irregularity might be. It is both an art and a mindset.

Kaizen inspired the fundamental thinking I put into the MedsPro® Medication Packing Solution. MedsPro arose from my desire to ensure we achieve the same accuracy in manually hand-packing medication as we get from the completely automated MedsPro® Packing Robot.

Successfully applying the Kaizen concept creates an environment of movement and rhythmic momentum. 

The first step in applying this concept to the development of the MedsPro solution was to make sure everything is in reach – no more than one-and-a-half steps. The one-and-a-half steps actually came from a staff member who played netball. For the technician preparing a ±ٱ-貹® or Unit Dose 7® pack, having everything they need within one-and-a-half steps, helps to foster a rhythmic momentum. With this rhythm comes productivity and quality output.

We have zero tolerance for error in pharmacy. We have a responsibility to ensure the protection of the community and those most vulnerable. The systematic – almost automatic – process of MedsPro helps cut packing errors to zero.

The MedsPro solution creates a productive packing rhythm with numbered and colour-coded shelving positions to store the medication. These features ensure the most frequently used medications are always closest to the packing technician to improve their flow of movement. Before MedsPro I remember watching staff packing out of the ‘integrity bags’. They were sitting and working diligently, no doubt, but I never really had a sense of how efficient or productive they were.

Now I can ‘feel’ the efficiency just by standing in their workspace. I can measure their productivity through the reporting and audit trails. I don’t have to guess. I can tell which staff are more productive than others. That matters when staff put in the effort to optimise their performance above and beyond others. Why should they not be acknowledged and rewarded?

Achieving and feeling the rhythm makes the work more interesting and actually reduces stress. It’s the same as the meditative concept of being in the moment. In my pharmacy we packed 574,000 individual doses of medication over a five-week period without even one ‘correction’ required. How DAAs are protecting the independence of patients and safeguarding quality medication management in home care.

Effective medication management plays a large part in maintaining safety and quality standards in home care. report found, 250,000 hospital admissions in Australia each year are due to medicine-related problems and half of these are preventable. Medication management is dictated by the policies, procedures and systems that care organisations have in place; but it can also be as simple as the efforts of family members and care providers, helping patients with their regular ±ٱ-貹®. Regardless of who is providing the support, it’s important to have tightly regulated conditions to protect against potential hazards related to mismanagement of medications.

Effective medication management largely impacts the independence of patients. The , states that patients should be encouraged to safely manage their own medications for as long as possible. By providing medication support and encouraging self-administration, we can delay the need for patients to be admitted to hospitals and residential aged care facilities, while also improving their well-being and sense of self proficiency.

Dose Administration Aids (DAAs) such as the ±ٱ-貹® medication pack, play an important role in medication management as they prompt and assist the user to take their medications correctly and on time. The key benefits of the Webster-pak medication pack include a colour patient photo, brightly coloured dosage times, individual blisters, a full list of medicines, and large fonts – also available in the patient’s native language. These carefully designed features prompt the patient or care provider when to take each medication.

Safety and quality guidelines dictate that unless care providers are registered nurses or supervised by a registered nurse, they are not able to administer, but only prompt and assist in the self-administration of medication. This is where DAAs, such as the ±ٱ-貹® medication pack, are valuable as they eliminate the need for care providers to make clinical decisions around medication administration. With the help of the ±ٱ-貹®, medication management is simplified to prompt and assist practices in line with correct medication support guidelines.

DAAs such as the ±ٱ-貹®, satisfy the extended Rights of Medication Support by ensuring administration;

  • To the Right Person
  • By the Right Route (mouth, eye, ear, skin)
  • Of the Right Number of Tablets
  • At the Right Time
  • Accompanied by the Right Documentation

The ±ٱ-貹® goes further to also address the Rights of Medication Administration that only Registered Nurses are qualified to complete to ensure the;

  • Right Dose

The ±ٱ-貹® simplifies the process of taking medications as its features address all the above criteria for safe administration. According to the Australian Nursing and Midwifery Federation (ANMF), all DAAs must be tamper proof and checked by a pharmacist, which means the integrity of the doctor’s medication order remains unchanged and safe for the patient. It also means it does not require a registered nurse onsite to check the correct medication and dosage before administration. To reduce risk of medication mismanagement or incorrect administration, you need to eliminate the need for care providers to make any clinical decisions when assisting their patients. DAAs achieve this by addressing all rights of medication support and administration. By using a ±ٱ-貹®, all care providers – even those without training such as family members, can prompt and assist the administration of medicines of the right amount, at the right time.

An effective medication management plan makes use of DAAs in line with a Home Medication Review by the Doctor. A Home Medication Review maintains best practices of administration, compliance and storage of medicines. The full management process also includes the use of a signing sheet for documenting when medication was given, and a PocketProfile™ to ensure a full list of all medications a patient is taking is readily available for all care providers.

In line with the Home Medications Review, the capacity of the individual to self-administer and manage their own medications must constantly be assessed. When reviewing the capacity of the individual, we determine the extent of their abilities and make a judgement on the degree of external assistance the individual needs to maintain safe medication management. It is important to note that with a DAA system such as the ±ٱ-貹®, safe management can be maintained at all levels of capability. Whether the patient can fully self-administer, if they need a family member or care provider, or even a registered nurse to assist them, ±ٱ-貹® allows you to administer the correct medication at the correct time.

At every possible opportunity patient independence must be protected by encouraging and supporting individuals to self-administer their medication. When safe medication management practices such as ±ٱ-貹® DAAs are in place, we can maintain the independence of elderly people and reduce the risk of hospitalization due to incorrect management of their medications.
